Please leave a message that includes your name, Student ID, date of birth, reason for appointment, and best contact number our office staff or therapist will contact you to schedule an initial appointment through Zoom.

We will continue to offer telephone triage services, which do not require an appointment. Please include your name, Student ID, date of birth, reason for consultation, and best contact number and a nurse practitioner will call you for personalized medical consultation.
